Passport Application (Can pick up in our office or fill it out online by clicking this link: Passport Application)
- Black ink only! No errors or white out.
- Sign application in front of agent.
- If under 16 both parents must accompany the child.
- All applicants including children must be present.
Expired passports (If Applicable)
Driver's License
Certified Copy of Birth Certificate from your State or Counties' registry.
Passport Photo (can be taken at CVS, Staples, Walgreens)
- Name written on back of chosen photo.
Personal Check or Money order made out to: U.S. Dept of State (Each passport needs their own money order or check)
Date of Birth written on check or money order.
Current Processing Times:
Routine: 6-8 weeks
Expedited: 2-3 weeks
